Eight-sided shape, the label is still intact, there is tape over it, made in the United States, empty, and does have chips on the bottom of the bottle (shown in photos). Approximately 4 3/4" tall, no lid and there is a residue feeling inside the bottle, I have not attempted to clean the inside. USED...PLEASE VIEW ALL OF THE PICTURES.
