It was a Man Of Steel steelbook pre-order exclusive from Wal-Mart in 2013. It has the original purchase card with it. Fits perfectly on the front of the steelbook, and is lenticular 3D, see photos (I tried to show in last photo what it looks like when you move it in the light). NO steelbook or discs are included, only the magnetic card cover. What you see in the photos is what you will get. Sorry for glare in photos.
